• After the username, enter your password of the account and then click onto the orange colored “Sign in” button to get into your account and get the services of the company.
• If you do not have an account on the company’s website, then you will have to click on the green colored “Sign Up Now” button to go to the registration form.
• Now, in the first field of the form, you will have to enter the MyUHealthChart activation code, if you do not have any activation code then click on the green colored “Request Online” button to get the code first.
• After the activation code, you have to enter your email address in the next field.
• After entering the email address, you will have to enter your date of birth in the last field of the form.
